15/10/2014:

"UNMEER: The United Nations Mission for Ebola Emergency Response (UNMEER) has announced a strategy for combating the Ebola outbreak. A target of the “70-70-60 plan” has been decided. This plan would ensure safe burial of 70 percent deaths and hospital-based isolation of 70 percent of suspected cases within 60 days of 1 October. UNMEER has cautioned that by 1 December, the detection of new Ebola cases could rise up to 10,000 per week. An urgent requirement of hospital beds and trained staff has also been highlighted."

Sierra Leone declared free of Ebola

7 November 2015

The World Health Organization has declared Sierra Leone to be free of Ebola. It is now more than 42 days after the last confirmed patient tested negative for the virus on 25 September and was discharged from a treatment centre. No further cases have been reported since.

In official totals, 8,704 people were infected with Ebola in Sierra Leone during this outbreak, however there are likely to be other cases which were not recorded. There were 3,589 fatalities, including 221 healthcare workers.

The country will maintain their vigilance during a 90 day period of heightened surveillance which runs until 5 February 2016. This will allow for early detection of any new cases which may arise, such as the detection of new cases linked to sexual contact with a survivor or importation from Guinea where there are still new cases being reported.

Science still has a very long road to run trying to eradicate Ebola, but it takes all in every country to work 101% in the knowledge already available.

These two cases demonstrate that when caring for mother-child pairs, healthcare workers should consider the potential role of testing relevant body fluids in addition to blood, such as urine and breast milk plus reference's to semen.

As the number of Ebola survivors continues to increase, there is increasing evidence that the Ebola virus can be detected in body fluids of survivors, such as in semen, breast milk, eye, placenta and the central nervous system, for longer than originally thought.

Both the World Health Organization (WHO) and the US Centers for Disease Control and Prevention (CDC) have issued press releases about a preliminary study published in the New England Journal of Medicine (NEJM). The study conducted in Sierra Leone has found fragments of the Ebola virus could be detected in the semen of one quarter of male survivors tested between seven and nine months post recovery. It was previously known that the virus could survive for up to three months in semen. It is unknown why the virus persists in some men while others clear it, nor how great their risk of infecting others is as time passes. Further tests are planned to determine the risk of transmission from other body fluids.

These findings will be used to update the WHO's interim advice on sexual transmission of Ebola. Ideally men should abstain from unprotected sex until they receive two negative test results from semen testing.

A second study published in NEJM has documented the first confirmed case of sexual transmission from a survivor in Liberia. A male survivor, whose blood twice tested negative for the virus was discharged from a treatment centre on 7 October 2014. He had unprotected sex with a woman on 7 March 2015, 155 days after the second negative blood test. She developed symptoms on 14 March and eventually died on 27 March. The virus was detected in his semen samples on 27 March, 175 days after his second negative blood test. The genetic sequence of the virus sampled from the survivor and the woman were matched. Although there has been much anecdotal evidence for sexual transmission of the disease, this is the first evidence of human to human transmission of Ebola virus through a sexual contact.

New Ebola case marks disease's return to Liberia

BBC Online

The body of a dead Liberian man has tested positive for Ebola - the country's first reported case since it was declared free of the disease.

Deputy health minister Tolbert Nyenswah said tests confirmed that the 17-year-old man, from a town near the main airport, had died of the disease.

Officials are investigating how he contracted Ebola, Mr Nyenswah said.

Liberia was declared free of Ebola six weeks ago, following the deadliest outbreak in the disease's history.

More than 11,000 people have died of the disease since December 2013, the vast majority of them in Liberia, Guinea and Sierra Leone.

The countries had largely curbed the spread of the disease - but the number of new cases has risen recently, with the start of the rainy season in West Africa.
